You can either fill out the .doc forms as Microsoft Word Documents on your computer then print them, OR print out the pdf forms and hand fill them out in ink. Either way you choose, bring the forms filled out to the office for your appointment to save waiting time.

One of our goals at Cayuga ENT is to provide you and your family with excellent medical care.   Another is to provide you with a pleasant and professional office experience.
